Minor Release Forms
There are two diffrent forms this year. Each parent must fill out one or the other form.
Each form is a two page form, one side for the parent to fill out and the other for the minor to fill out.
One form is for parents and minor signing at the track in the presence of a track official.
The other form is for the minor and one or both parents that can not be at the track. This form must be notorized.

Minor Release Forms can be downloaded and printed here. To be a legal document it must be printed in color and can NOT be folded.
Minor Release Forms can also be picked up at the Fair Office.
Black and white or folded documents will not be accepted.
